Common Treatment Indications for Hyperbaric Care

Indications and Limitations for Hyperbaric Coverage

The list of indications and associated explanations has been taken directly from the websites of the Centers of Medicare & Medicaid Services. Most commercial insurance providers will follow the CMS guidelines. Program reimbursement will be limited to that which is administered in a chamber (including the one-man unit) and is limited to the following conditions:


  • Acute carbon monoxide poisoning
  • Decompression illness
  • Gas embolism and gangrene
  • Acute traumatic peripheral ischemia
  • Crush injuries and suturing of severed limbs
  • Progressive necrotizing infections
  • Acute peripheral arterial insufficiency
  • Preparation and preservation of compromised skin grafts (not for primary management of wounds)
  • Chronic refractory osteomyelitis (unresponsive to conventional medical and surgical management)
  • Osteoradionecrosis (adjunct to conventional treatment)
  • Soft tissue radionecrosis (adjunct to conventional treatment)
  • Cyanide poisoning
  • Actinomycosis (adjunct to conventional treatment when disease process is refractory to antibiotics and surgery)
  • Diabetic sounds in the lower extremities (under certain conditions)


In general, hyperbaric is covered as an adjunctive therapy only after there are no measurable signs of healing for at least 30 days. Most insurers require standard wound therapy and wound care first. If no measurable healing occurs for at least 30 consecutive days, hyperbaric may be indicated. Wounds must be evaluated at least every 30 days during administration of hyperbaric. Continued treatment with hyperbaric is not covered if measurable signs of healing have not been demonstrated within any 30-day period of treatment.


Treatment for Endangered Limbs

Hyperbaric is one of the best options for limb injuries. Patients with acute traumatic peripheral ischemia can benefit from hyperbaric as an adjunctive treatment. When used alongside other therapeutic measures, there is a reduced risk of loss of limb, loss of function, or even loss of life. Moreover, it is an indicated treatment for crush injuries or suturing of severed limbs. As with the previous conditions, hyperbaric can be used as an adjunct in order to boost better patient outcomes.


These injuries can be devastating for patients. Providing additional support to promote better outcomes is the superior option.


Indications for Diabetic Care

At LST Group, we know that hyperbaric is indicated for diabetic wound care in some cases. In particular, it is useful in wounds of the lower extremities. For treatment to be covered, the following three conditions must be met:


  • Patient has type I or type II diabetes with a lower extremity wound caused by the disease
  • Patient has a wound classified as Wagner grade III or higher
  • Patient has failed an adequate course of standard wound therapy


If these three conditions are met, hyperbaric is an indicated treatment option. LST Group can equip your medical facility to provide such care. Standard wound care in patients with diabetic wounds includes: assessment of a patient’s vascular status and correction of any vascular problems in the affected limb if possible, optimization of nutritional status, optimization of glucose control, debridement by any means to remove devitalized tissue, maintenance of a clean, moist bed of granulation tissue with appropriate moist dressings, appropriate off-loading, and necessary treatment to resolve any infection that might be present.
